Application for certification

1. The application for certification shall include, at a minimum:

1) the name of the entity applying for certification or its name and an indication of the address of its registered office, the address of its place of business or the address of its residence;

2) information confirming compliance with the certification criteria;

3) indication of the scope of the requested certification.

2. The application shall be accompanied by documents confirming compliance with the certification criteria, or copies thereof, and, in the case of certification by the President of the Authority, proof of payment of the fee referred to in Article 26.

3. The application shall be submitted in writing in paper form bearing a handwritten signature or in electronic form bearing a qualified electronic signature. An application submitted to the President of the Authority in electronic form shall bear a qualified electronic signature or a signature confirmed by the ePUAP trusted profile.
