Application for accreditation to monitor compliance with the approved code of conduct

1. Accreditation of the entity referred to in Article 28 shall be granted upon application, which shall include at least:
1) the name of the entity applying for accreditation and the address of its headquarters;
2) information confirming compliance with the criteria referred to in Article 41 (1) and (2) of Regulation 2016/679.
2. The application shall be accompanied by documents confirming compliance with the criteria referred to in Article 41 (1) and (2) of Regulation 2016/679, or copies thereof.
3. The application shall be submitted in writing in paper form bearing a handwritten signature or in electronic form bearing a qualified electronic signature or a signature confirmed by an ePUAP trusted profile.
