Notice to the President of the Authority of the appointment of an inspector
1. The entity that appointed the inspector shall notify the President of the Office of his appointment within 14 days from the date of appointment, indicating the name, surname and e-mail address or telephone number of the inspector.
2. The notification may be made by a proxy of the entity referred to in paragraph 1. The notification shall be accompanied by a power of attorney granted in electronic form.
3. In addition to the data referred to in paragraph 1, the notice shall indicate:
1) name and address of residence, in case the controller or processor is an individual;
2) the company name of the entrepreneur and the address of the place of business, in case the controller or processor is a sole proprietor;
3) full name and registered office address, in case the controller or processor is an entity other than those indicated in items 1 and 2;
4) REGON identification number, if assigned to the controller or processor.
4. The entity that appointed the inspector shall notify the President of the Office of any change in the data referred to in paragraphs 1 and 3, as well as the dismissal of the inspector, within 14 days of the occurrence of the change or dismissal.
5. In the case of the appointment of a single inspector by public bodies or entities or by a group of entrepreneurs, each of these entities shall make the notification referred to in paragraphs 1 and 4.
6. The notices referred to in paragraphs 1 and 4 shall be prepared in electronic form and bear a qualified electronic signature or a signature confirmed by an ePUAP trusted profile.
