Right of access of data subject

1. The data subject is entitled to obtain confirmation from the controller as to whether or not personal data concerning him or her is being processed, and if this is the case, he or she is entitled to access it and the following information:

a) cele przetwarzania;

(b) categories of relevant personal data;

(c) information on the recipients or categories of recipients to whom personal data have been or will be disclosed, in particular on recipients in third countries or international organisations;
(d) where possible, the planned period of storage of personal data and, where this is not possible, the criteria for determining that period;
(e) information on the right to request the controller to correct, delete or restrict the processing of personal data concerning the data subject and to object to such processing;
(f) information on the right to lodge a complaint with the supervisory authority;
(g) where personal data have not been collected from the data subject any available information about their source;
(h) information regarding automated decision-making, including profiling, as referred to in Article 22(1) and (4), and—at least in those cases—meaningful information regarding the logic involved, as well as the significance and the envisaged consequences of such processing for the data subject.
2. If personal data is transferred to a third country or an international organization, the data subject has the right to be informed of the appropriate safeguards referred to in Article 46 relating to the transfer.

The controller shall provide the data subject with a copy of the personal data being processed. For any subsequent copies requested by the data subject, the controller may charge a reasonable fee based on administrative costs. If the data subject requests a copy electronically and does not specify otherwise, the information shall be provided by commonly used electronic means.
4. The right to obtain a copy referred to in paragraph 3 shall not adversely affect the rights and freedoms of others.
